We deliver the My Wellbeing Hub Digital Platform and work with other organisations and partners to make sure that you receive all the services you need, when you need them. We want to focus on your needs to make sure that you get the right support to assist you on your wellbeing journey.

Your name; date of birth; address, contact telephone number and contact email address

Gender

Details around your health including Mental health, Physical health, Social wellbeing, Mindset and attitudes to health, Motivation, Confidence and esteem, Health Conditions, Disability.

This data is also shared with MegaNexus Limited who are responsible for the development and maintenance of the platform and as such have access to data as part of their enhanced access to the platform,

We will only share your personal information for reasons mentioned above unless the law says we are required to share it for another reason, or as believe we must share the information to protect you or others.

Your personal information will be stored securely and retained in line with the platform’s privacy policy. Your data may be stored for a longer period if required for audit purposes.
