Starting in January 2026, we are launching an innovative Long-Term Conditions Prevention Service in Greater Manchester and North East & North Cumbria, commissioned by NHS England as part of the National Diabetes Prevention Programme (NDPP). This pilot will help thousands of people reduce their risk of serious health conditions through personalised lifestyle support.
Why it matters
Long-term conditions such as hypertension and cardiovascular disease are among the biggest health challenges facing the UK. Research shows that simple lifestyle changes can significantly reduce these risks. Building on the success of the NDPP, this new service expands support to include people with high blood pressure.
Evidence behind the programme
A large observational study assessed the incidence of 35 long-term conditions, including Type 2 diabetes, up to 24 months after participants started the NDPP. Results showed that individuals who completed the NDPP had a significantly lower risk of developing conditions such as diabetes and cardiovascular disease compared to those who did not attend. This demonstrates the power of lifestyle interventions in improving long-term health.
Aims of the service
Reduce the incidence of long-term conditions (LTCs).
Support people to achieve or maintain a healthy body weight in line with UK dietary guidelines.
Promote healthy eating habits: fibre, fruit and vegetables, oily fish, and reduced saturated fat, salt, and free sugars.
Encourage increased physical activity and reduced sedentary behaviour, aiming for Chief Medical Officer (CMO) recommendations.
Support offered via face-to-face groups, digital platforms, and remote sessions.
Who can join
This pilot programme is specifically for individuals diagnosed with hypertension. Eligible participants must:
Be aged 18 years or over.
Have a recorded diagnosis of hypertension in their GP records.
